Department Description
This position is in the Center for Excellence in Community Mental Health (CECMH), which provides clinical and rehabilitative services to persons with mental illness. The Center provides psychiatric and psychosocial assessment, medication management, case management, psychotherapy, family services and other community mental health services. The Center is also a clinical training site for trainees from multiple disciplines and collaborates on relevant research projects.
Position Description
The primary purpose of the Care Manager is to provide integrated physical and mental health care management for Center patients as part of the Integrated Care Program. The Care Manager will monitor transitional care for Center patients. The care manager will be expected to serve patients in office and community-based settings, to fulfill care management duties. The care manager will follow patients with chronic physical and mental health concerns and perform medication reconciliation, care coordination, disease education, and disease self-management. The care manager will also collaborate with data collection efforts to track physical and mental health disease outcomes.
Minimum Education and Experience Requirements
License to practice as a Registered Nurse in the State of North Carolina.
Preferred Qualifications
Data collection and some data entry; ability to apply nursing skills, knowledge, and abilities to chronic disease education, medication adherence, disease self-management, and care coordination; communicate program activities with Center staff; provide disease education information for staff; ability to effectively function on a team with the Center Primary Care Clinic RN, Clinic Manager, Program Director, and Program Manager to review program goals and activities; attend regular meetings with program leadership; submit summaries of clinical data; assist with primary care clinic RN duties as needed; ability to use Microsoft Office and Microsoft Outlook programs; ability to use electronic medical record systems.
